어제 ‘생명과희망’ 회원님의 CT 판독에 대한 해석은 저에게 큰 도움이 되었습니다.
앉으면 눕고 싶은 것이 인간의 욕심이라 했던가요. 한 번 더 부탁을 올리겠습니다.
제가 4월 말에 CT상에 이상이 발견되어 이를 좀 더 면밀하게 추적 관찰하고자 약 3개월 간 CT 3회, PET/CT 1회 검사를 받았습니다. 담당 의사께서도 성실히 서령을 하려 했겠습니다만 궁금한 점이 한 두 가지가 아니더군요. 이에 관련한 판독자료 해석을 한 번 더 부탁드리겠습니다. 다시 한 번 감사하다는 말씀을 올립니다. 감사합니다~^^

■ Date 2020-04-29
CT. Liver dynamic(with enhance) (No oral water)
▷History:
1.CBD cancer로 수술한 환자임
2. 2020-1-9 CT와비교하여 판독함.
▷Technique:
Precontrast 및 Intravenous contrast material 주입 후 동맥기, 정맥기 및 3분 지연기의 triphasic dynamic CT scan을 시행 함.
▷Finding:
Liver attenuation 낮으며 fatty liver disease가 있는 것으로 보임.
Liver Segment VI에는 AP shunt에 의한 것으로 생각되는 enhancement가 있음.
Liver에 뚜렷한 focal lesion 없음
복강내에 significant lymphadenopathy 없음.
Right kidney의 renal cyst 변화 없음
이전 CT에서 명확하지 않던 omental nodule과 haziness가 의심됨.
Peritoneal carcinomatosis 가능성 있는 소견으로 생각됨
▷결론
-. Peritoneal carcinomatosis, highly suspected.
-. Fatty liver disease.

-. 2020-04 CT에서 언급한 omental nodule과 Peritoneal haziness 병변들 보이나 뚜렷한 hypermetabolism 없음(IM #137, 138, 144, ...).
-. Bowel loop을 따라 multifocal & diffuse hypermetabolic activity 있고 투약력 참조시 metformin 복용에 의한 physiologic uptake임.
-. Both mid-to-lower lungs의 multiple nodular lesions 큰 변화 없어 보임(IM #87, 94, 106, 109, ..).
-. 그 외 전신 scan field 내에 physiologic activity 제외한 significant hypermetabolic lesion 없음.
▷결론
1. No significant hypermetabolic activities in small omental nodules & Peritoneal haziness; Probable reactive change more likely than low metabolic peritoneal seeding
2. Otherwise, no significant hypermetabolic lesion in whole body

CT, Liver dynamic & Pelvis(with enhance)
▷History:
1.CBD cancer로 수술 받았음.
2.Peripheral T-cell lymphoma로 치료받았음.
3. 2020-04-29 CT와 비교함.
▷Technique:
Precontrast 및 intravenous contrast material 동맥기, 정맥기의 four(or two) phasic liver dynamic CT 시행하고, 이어 abdomen과 pelvis에 대하여 scan함
▷Finding
-. 이전 CT에서 언급된 Omental nodule과 haziness lesion들이 약간 더 prominent 해져 peritoneal carcinomatosis 가능성이 있겠음(SE 6 IM 35, 43, 45, 47).
-. Peritoneal thickening도 약간 더 prominent해 졌음.
-. Ascites는 없음.
-. Liver에 전이를 시사하는 focal lesion.
-. Bile duct dilatation 없음.
-. 복강내에 다른 새롭게 생긴 전이 소견 없음
-. Scan된 범위의 lower lung에 lung nodules은 2020-04-29 chest CT와 비교하여 변화 없음.
▷결론
Slightly prominent of peritoneal seeding lesions - probable carcinomatosis Peritoneal.
■Date 2020-07-08
CT. Abdomen & Pelvis(with enhance)
▷History:
-. CBD cancer와 lymphoma로 follow up중인 환자임.
-. 2020-6-6 CT와비교하여 판독함.
▷Technique:
Oral(water) 및 intravenous contrast material 주입 후 routine abdomen 및 pelvic CT protocol로 시행함.
▷Finding:
-. Segment IV에 segmentectomy와 biliary enteric anastomosis가 시행되어 있음.
-. Op bed 주변으로 soft tissue density(SE 2 IM 23)있고 postop change로 판단함.
-. Liver에는 뚜렷한 recurred tumor 보이지 않음.
-. Pneumobilia의 소견 있음.
-. Omentum에 multiple omental nodule들이 보이고 있으며 peritoneal seeding의 가능성이 있을 것으로 판단되는 병변임.
-. Ascites, pelvic cavity에 소량 있음.
▷결론
1. Equivocal change of soft tissue density, remnant liver, (hilum??====), R/O postop change.
2. Equivocal change of nodular infiltrations in the omentum, R/O peritoneal seeding.
★★★결론 1을 “1.Equivocal change of soft tissue density, remnant liver hilum, R/O postop change.로 수정함”
