Gastrointestinal stromal tumor, high risk of malignant potential (NIH 2001),
Stage IIIA (T4/low mitosis)(AJCC 7th edition)
1) tumor size : 12.8x8.7x4.0cm
2) mitosis: 3/50HPFs, Ki-67 proliferation index: 5%
3) C-kit(+)/CD34(-)/SMA(+)/desmin(-)/S-100 protein(-)/DOG-1(+)
4) mainly in subserosa with invasion to muscle proper
- adhesion to pancreas
5) spindle and epithelioid cell type with moderate to marked nuclear atypia/
moderate increased cellularity
6) necrosis : present (20%)
7) no definite lymphovascular tumor emboli
8) clear resection margin
GROSS :
Submitted is a product of segmental resection of proximal jejunum, measuring 13.2cm in length and 7.20 m in circumference. On open, it shows a relatively well-defined exophytic mass at serosa, measuring 12.8x8.7x4.0cm in dimensions. The cut surface of the mass is grey brown and variegated, and shows fo ci of myxoid degeneration, hemorrhage and necrosis. The mass is 1.8cm and 0.2cm aprat from the res ection margin #1 and #2, respectively. Sections are blocked in A to K.
